2/15/2023 EnchantaVaughn Approved Behavior: Client was active and engaged when QMHP arrived at the residence. Client was witnessed smiling and interacting well with his mother and brother. Client was very polite and communicated happy thoughts around his family. Client participated in his therapy without getting frustrated or acting out. Client was receptive to QMHP prompts and directives. Client was open to sharing his feelings and the things he believed he needed assistance with. client continues to struggle with expressing self without reverting to Anger. Behavior: Client displays understanding of his strengths and weaknesses. Client mood was viewed as patient and laid back. The member has been struggling with personal space as it relates to others; session focused on personal boundaries to give others space and knowing when to be assertive. Intervention: QMHP conducted the session to introduce activities that focused on parenting and journaling. QMHP reviewed with client what trust means and why it is important to be honest and trustworthy. QMHP actively listened and provided support to client when needed. QMHP inquired about how client was feeling. QMHP made sure client was comfortable and in a good space before introducing him to a new therapeutic skill. QMHP revisited effective communication skills and positive verbal affirmations with client to help him express himself in an open way. Intervention: QMHP went through program overview with client and mom to reiterate key points and program objectives that would pertain to client specific wants and needs. QMHP retrained client on relaxation skills, which included deep breathing, muscle relaxation, and calming imageries as a way for client to regain and control his rebellious and hyperactive conduct. QMHP reiterated to the client that he doesn’t have to be so hard on himself and that he should give himself some credit for surviving a life crisis. Response: Client struggled with expressing his feelings in response to the negative energy around him. Client frowned his face up and ignored QMHP prompts when directed to follow his mother’s directions. Client failed to identify positive ways to deal with his anger and frustration, remaining aggravated the entire session. Client continued to express negative statements to QMHP. Client reduced eye contact with QMHP and showed no interest in following safety plan. Client offered no form of verbal communication and refused to calm down. Response: The client participation level was at an all-time low. The client looked confused, indicating that he didn’t know the reason for his negative behaviors. Client understood his expectations and what ways he could utilize the session to best fit his transition through his crisis. Client shared some of his needs. Client was open to new interventions and promised to let his mom and counselor know when he felt overwhelmed and needed a break/cool down from everything.
